If you are a computer reseller and would like to purchase from CWI, download either newacct.pdf or newacct.zip.  If you downloaded newacct.pdf then use Adobe Acrobat Reader to print it out.  If you downloaded newacct.zip then use your favorite zip utility to extract newacct.doc to a location of your choosing.  Once you have extracted newacct.doc, then use Microsoft Word or Word Pad to view and print the file.  Regardless of the method you use, once the forms are printed out, fill them out and then fax them back to us with a copy of your state tax exempt forms.
